Mississauga Urology Associates ("we", "us", "our") is committed to protecting the privacy of our website visitors and prospective patients. This policy explains what personal information this website collects, how it is used, who it is shared with, and the choices you have, in accordance with the federal Personal Information Protection and Electronic Documents Act (PIPEDA).

• Contact form submissions: full name, date of birth, phone number, email address, the doctor you selected, and any message you choose to write.
• Basic technical information collected automatically by our hosting and security providers (such as IP address and browser type) as part of operating and securing the site.
• Standard website usage data if and when analytics tools are enabled (see "Cookies" below).
• No medical records, test results, or clinical documentation are collected or stored through this website.

We do not sell or trade your personal information, and we do not share it for marketing purposes. To operate this website and the contact form, a small number of service providers process your information on our behalf:

• FormSubmit - the contact form is delivered to our clinic inbox through FormSubmit, a third-party form-relay service. The name, date of birth, phone, email and message you submit pass through FormSubmit's servers in order to reach us. Please do not use the form to send detailed medical history, test results or other sensitive health information.
• Cloudflare Turnstile - used to confirm you are not an automated bot before the form can be submitted. Cloudflare processes limited technical data (such as your IP address) to do this, under Cloudflare's own privacy policy.
• Google Maps - the clinic location map embedded on this website is loaded directly from Google and is subject to Google's privacy policy.
• We may also disclose personal information where required by law, court order, or to protect the safety of a patient or the public.

• To facilitate appointment and referral requests
• We do not use contact form information for marketing or newsletters

• Contact form submissions are transmitted over an encrypted (HTTPS) connection.
• This website does not use browser localStorage or sessionStorage to store personal data.
• The contact form is not a secure patient portal or encrypted messaging system. Please do not send detailed medical history, images, or test results through it - call our office instead for anything sensitive or urgent.

We keep contact form submissions only as long as reasonably necessary to respond to your inquiry and maintain a record of that communication, after which they are deleted or archived. If you become a patient, any information relevant to your care becomes part of your medical record and is retained under PHIPA and our regulatory obligations, not under this website policy.

This website does not currently run analytics or advertising cookies. The Cloudflare Turnstile bot-check and the embedded Google Maps widget may set their own limited technical cookies as part of how they function. If we add analytics (such as Google Analytics) in future, we will update this section and, where required, ask for your consent first.

This website contains links to external resources, including the Canadian Urological Association, YouTube, and device manufacturer websites. Those websites have their own privacy practices, which we do not control and are not responsible for.

You have the right to access the personal information we hold about you, ask us to correct it, and withdraw your consent to our continued use of it, subject to legal and contractual restrictions. To make a request, use our contact form and mark it "Attn: Privacy Officer."

If you are not satisfied with our response, you may complain to the federal Office of the Privacy Commissioner of Canada (priv.gc.ca), or, for concerns about health information specifically, the Information and Privacy Commissioner of Ontario (ipc.on.ca).
